Oak Street Health is a rapidly growing company of primary care centers for adults on Medicare in medically-underserved communities where there is little to no quality healthcare. Oak Street’s care is based on an entirely new model that is based on value for its patients, not on volume of services. The company is accountable for its patients’ health, spending more than twice as long with its patients and taking on the risks and costs of their care. Since 2013, Oak Street Health has been investing in communities across Illinois, Indiana, and Michigan – providing much-needed primary care for tens of thousands of people
Since our founding in 2012, the mission has been to build a primary care delivery platform that directly addresses rising costs and poor outcomes, two of the most pressing challenges facing the United States healthcare industry. Older adults have specific healthcare needs. Many don’t receive it. When they do, appointments are brief, in-network doctors can be far away and they don’t get personalized treatment plans. Oak Street is changing all of that.
Nothing is more personal than your health. Our care is personal, spending significantly more time with our patients and knowing them as individuals. You should have access to the same great care no matter where you live. We invest in our communities by building Oak Street Health centers where healthcare services are needed most.
Many adults on Medicare have chronic and complex conditions. We hold ourselves accountable for our patients’ health by taking on the risk and cost of their care.
Amanda Kirchner has been the Outreach Director for Oak Street Health in Rockford since June 2017. From August 2007 to June 2017, Amanda served as Vice President of Business Development at Workflo a solution by beyondRelations, LLC in Rockford, a software company that develops, sells and implements software for the aerospace supply chain. And from October 1998 to June 2007, she was the Customer Relations and Special Operations Coordinator for PulteGroup in Huntl1y, Illinois.
Amanda received an AAS degree in Construction Technology from Rock Valley College. In her spare time she has been active with Womanspace, serving as Lunafest Chair, Treasurer of the Board of Directors, and as Chair of the Finance and Development Committee.
Amanda will talk about Oak Street's history in Rockford and share some of the services that they provide for the elderly in our Community. This should be of interest to many of our members who would fall into that category of "elderly". This will be a relevant and informative program, one in which we encourage our members to invite a guest or prospective member to join our Zoom meeting.
